Medco — Clinical Notes & Live Transcription

Problem
Clinics needed to capture doctor–patient conversations during appointments and turn them into structured, role-appropriate clinical notes in real time.
Built
  • Architected the full-stack app end to end with two roles (clinical assistant and doctor) and appointment scheduling — hospital, doctor availability, and new-patient onboarding.
  • Built continuous live transcription: audio is streamed in ~5-second chunks and merged with an overlapping-transcription algorithm that reconciles partial, incomplete segments into one continuous transcript.
  • Generated the structured clinical notes from that transcript via an LLM model API call.
  • Role-based visibility driven by pre-defined configs — doctors control what assistants can see; doctors see all notes, assistants only the permitted categories.
  • Real-time collaboration over WebSockets: assistant and doctor join the same live session and see notes according to their role.
  • Surfaced the patient's diagnosis history (captured at booking) inside the appointment session tab.
  • Scaled WebSockets across multiple FastAPI workers using Redis, and handled the Nginx deployment configuration.
